Our Office Policies

​

Welcome and thank you for allowing us to be of service to you. You will be asked to complete some paperwork before your appointment.  You may complete the forms at our office prior to your first appointment. We ask that you arrive at least 20 minutes early to allow for enough time.

There are a few other pieces of information that will also help with your experience here. Any previous evaluations that have been completed, IEPs, 504 Plans, medication lists or other relevant health information will allow our clinicians to more easily develop a treatment plan. We will also need to make a copy of your insurance card and identification. This is a fraud prevention precaution that the health insurance companies require from us for your protection.

We will do our best to obtain as much detailed information about your insurance plan and any associated fees such as copays or deductibles prior to your appointment. You may also check with your insurance carrier to find out this information. We look forward to working with you and hope you have a pleasant experience at our office.

Appointments/Cancellations

With the exception of serious emergencies, it is expected that you keep all of your appointments. We ask that you notify us at least 48 hours in advance if you need to cancel. In such a case, please call our office and arrange for a make-up appointment.

Emergencies

Our office provides on-call service for emergencies. Call 401-290-7901 so that your clinician on call can be contacted.
